These Terms apply together with our Privacy Policy, treatment-specific consent forms, and any written quotation or service agreement we provide.

Revival is a doctor-led medical and aesthetic practice located at Second floor Room B3, Health and Wellness Center, 353 Rivonia Blvd, Rivonia, Sandton, 2191, South Africa.
Our services may include, without limitation:
- General practice and primary healthcare
- Medical aesthetic treatments (for example, injectables, skin rejuvenation, and body contouring)
- IV wellness drips and related supplementation
- Hair-loss assessment and treatment

Services are subject to clinical assessment. We reserve the right to decline or discontinue treatment where it is not medically appropriate, safe, or in accordance with professional standards.
3. Eligibility and patient responsibilities
You agree to:
- Provide accurate, complete, and current personal and medical information
- Disclose relevant medical history, allergies, medications, pregnancy or breastfeeding status, and prior treatments
- Follow pre- and post-treatment instructions provided by our clinicians
- Attend scheduled follow-up appointments where advised

- Behave respectfully toward staff, practitioners, and other patients
Our services are intended for adults unless otherwise agreed in writing for paediatric or dependent care with appropriate guardian consent. You must have legal capacity to enter into these Terms, or have a parent or legal guardian agree on your behalf.
4. Appointments, cancellations, and late arrivals
Appointments may be booked by telephone, email, in person, or through channels we make available from time to time. A booking request does not guarantee an appointment until confirmed by Revival.
If you need to cancel or reschedule, please provide reasonable notice. We may apply a cancellation or no-show fee where repeated late cancellations or failures to attend occur, or where specialist products, time, or staff have been reserved for your appointment. Any such fees will be communicated to you in advance where practicable.
If you arrive late, we may need to shorten or reschedule your appointment to avoid delaying other patients. Deposits or pre-paid amounts may be non-refundable where stated at the time of booking.
5. Consultations, consent, and medical decision-making
All treatments begin with an appropriate consultation and clinical assessment. You will be informed of the proposed treatment, expected benefits, material risks, alternatives, and estimated costs before proceeding, where applicable.
You must sign any required consent forms before treatment. You may withdraw consent at any time before a procedure begins, subject to any costs already incurred.
Nothing on our website replaces a face-to-face medical consultation. Online information is general in nature and may not apply to your individual circumstances.
6. Treatment outcomes and medical disclaimer
Medical and aesthetic results vary between individuals. While we use evidence-based practices and professional judgment, we do not guarantee specific outcomes, longevity of results, or complete resolution of any condition.
Complications, side effects, or unsatisfactory results can occur even when treatment is performed correctly. You accept that aesthetic and medical procedures carry inherent risks, which will be explained during consultation.

7. Fees, pricing, and payment
Prices displayed on our website, price list, or marketing materials are subject to change without notice and may exclude applicable taxes, consumables, or additional sessions unless expressly stated.
Final fees may depend on your clinical assessment, treatment plan, and products used. You are responsible for payment at the time of service unless alternative arrangements are agreed in writing.
Where medical aid or third-party payment applies, you remain responsible for any co-payments, shortfalls, or rejected claims unless we agree otherwise.
We may use third-party payment processors. By making payment, you agree to their applicable terms where relevant.
8. Refunds, credits, and packages
Consultation fees, dispensed medication, opened products, and completed treatments are generally non-refundable, except where required by applicable consumer protection law or at our discretion in exceptional circumstances.
Treatment packages, courses, or prepaid plans are subject to the specific terms provided at purchase, including validity periods and transferability. Unused portions may expire or be forfeited if terms are not met.
Refund or credit requests must be submitted in writing with supporting details. We will assess each request fairly and in accordance with these Terms and applicable law.
